The ideal candidate will be able to create a comfortable andengaging environment tailored to the specific needs of each individual. Wehave several people in the community seeking home providers, would you beinterested in helping them find their forever home?
• Male client #1 is an enthusiastic Denver Broncos fan who enjoys traveling, especially to visit his brother in Wichita. Support this individual in baking, watching TV, and going out to eat, with a preference for burgers. Also providing companionship and share his love for breakfast and a good cup of coffee in the morning.
• Male client #2 is a creative homebody who enjoys spending time outside under a tree. Accommodate Jakes need for space with a basement room in a quiet location. Support his passion for drawing and provide a supportive environment for his creative expressions.
• Male client #3 enjoys going for walks, attending church, and visiting family regularly. Assisting him in maintaining a clean living space while respecting his preference for a quiet environment. He would do well with a family without children, as he values his privacy.
• Female client #1 values calmness and privacy, preferring a quieter environment. Encouraging her to go out while respecting her need for alone time in her bedroom. Seeking a patient and understanding individual or a mother with smaller family to be her Shared Living Provider.For nearly 40 years, Mosaic has provided improved quality of life, greaterstability and increased community access for people with disabilities throughour shared living service, which also offers a satisfying career option forhome providers.As a home provider, you share your home with an individual with a disability.Your responsibility is to maintain a safe and healthy home environment for theindividual and be a mentor while helping the person as needed with the tasksof daily living.In this role, you assist with cultivating life skills and fosteringindependence. We work to match interests and lifestyles between people. Youwill participate together in activities of daily living, both at home and inthe community. Through the matching process, you are able to match with aperson whose needs fit the support level you wish to offer. Duties vary foreach individual from minimal support (i.e., budgeting, medication,transportation, activities of daily living) to full support (i.e., dressing,toileting, feeding, positive behavioral support, etc.), and the income iscommensurate with the level of support provided.
• Responsibilities include :
• Provide support for tasks such as cooking, cleaning, laundry, budgeting, decision-making, etc.
• Skill acquisition
• Medication administration
• Medical appointments
• Support with overall health and well-being
• Daily documentation via web-based case management application (THERAP)
• Provide fun and leisure experiences to the individual by providing opportunities of community inclusion, integration and socialization
• Compliance with all stateagency policy and procedures
• Communicate effectively with team members as needed
• Support physical needs of individual served
• Provide transportation
• Possible responsibilities depending on your preference :
• Assistance with bathing, toileting, feeding, behavioral support, etc.
• Provide support for medical protocols, such as seizure, fall, feeding tube, etc.
• Care for someone with complex physicalmedical needs (i.e., hearing impaired, vision impaired, non-ambulatory, etc.)

• Must be a resident of the state in which you wish to provide contracted services in
• Transportation, valid drivers license, car insurance and current registration
• Active homerenters insurance

• Reliable computer and internet access
• Complete all state mandated trainings
• Environmentally safe and clean residence
