Job Summary : MRI Technician position at Mount Carmel Grove City, Ohio.
Work Schedule : Days M-F 1500-2330.
Job Requirements
BLS certification required for all direct patient care personnel.
License / Certificate Verification must be dated within 30 days of start.
Fit Test required prior to start.
Trinity 9 Panel Drug Screen must be completed within 30 days of start date.
Trinity Code of Conduct Form must be signed and on file.
Trinity I9 must be completed by the agency.
Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed and on file.
Trinity Integrity and Audit Services Form must be completed.
Trinity Specialty Specific Competency Testing must be completed within a year.
Trinity Color Vision test required.
2 Step PPD OR IGRA (Quantiferon / Tspot) required.
Trinity Specific Covid Questionnaire must be completed.
Trinity Skills Checklist required for clinical contractors.
COVID-19 Vaccine proof required.
MMR vaccination proof required.
Hepatitis B vaccination proof required.
Influenza Vaccine proof required.
Varicella vaccination proof required.
Tdap vaccination proof required.
Physical / Health Statement required.
Trinity Profile including resume and references required.
Trinity Conflict Of Interest Disclosure Form must be signed and on file.
Trinity Criminal Background Check must be completed within 30 days of start.
Trinity Education Verification required.
Trinity EPLS report must be attached and dated within 30 days.
Trinity GSA / OIG report must be attached and dated within 30 days.
Trinity NSO report must be attached and dated within 30 days.

Additional Details
American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS. Must include an image of the front and back of the card.
