JOB SUMMARY:
The Case Manager II Behavioral Housing provides lead support to the Housing Case Managers of Supportive Housing. In addition to housing case manager oversight, this position is responsible for assisting the Coordinator of Behavioral Housing with hiring and training of case management staff for housing. The Case Manager II Behavioral Housing also provides case management activities such as monitoring, referral, application for entitlements, illness education, supportive counseling, community integration, assessments, and coordination of service delivery. Provides direct and indirect, periodic service consisting of assessment, person- centered planning, referral/linkage, and monitoring/follow up on behalf of individuals receiving services. This service may be provided in multiple locations and various environments such as home, school, and community.
ESSENTIAL FUNCTIONS:
- Provide lead support to the Housing Case Managers of Supportive Housing. Assist the Coordinator of Behavioral Housing with hiring and training of case management staff for housing. Provide case management services to individuals who meet eligibility criteria in supportive housing settings; provide assessment of the individual's functional capacity, determine needs, and develop person-centered treatment goals/plans based upon assessment as well as monitors outcomes of person-centered plan for effectiveness, revising plan as needed.
- Function as a coordinator of service delivery to ensure that individuals/family members receive effective and appropriate treatment; makes referral and provides advocacy when indicated; monitors delivery of all services to ensure quality of care as well as continued appropriateness of service(s) including location where services/supports are provided.
- Assist served individuals in developing problem solving skills and activities of daily living which promote community integration within the housing setting; provide education, supportive counseling, and monitoring to prevent decompensation of serious mental illness and to maintain supportive housing status.
- Responsible for referral and linkage activities which connect an individual with medical, behavioral, social, and other programs, as well as services, entitlements, and supports to address identified needs and achieve goals specified in the Person-Centered Plan (PCP).
- Assist served individuals who are hospitalized with discharge planning needs and community/housing re-integration. Coordinates and promote the use of natural supports in planning services/supports with the individual/guardian/legally responsible person and the treatment team.
- Document all service delivery in the electronic health record; comply with policies and procedures established by Mount Rogers CSB, Licensure, Medicaid, and other regulatory bodies; adhere to confidentiality standards accordingly.
- Actively participate in meetings such as supervision, clinical staffing meetings, and committees as assigned; utilize these meetings for active problem solving and to enhance a team approach to decision making: keep mandatory trainings up-to-date and attend training to increase case management knowledge, skills, and abilities.
- Conduct in-home visits at least monthly and as needed to assess housing stability and progress towards supportive housing goals.
OTHER DUTIES:
- Participate in ongoing Quality Improvement activities including peer review,
- Provide high quality customer service, serving as a positive representative for the Agency,
- Perform such other duties as assigned by supervisor.
QUALIFICATIONS:
- Must be available on an as-needed basis for consultation and an on-call basis to assist in problem solving or addressing specific concerns;
- Knowledge of treatment modalities and interventions such as service coordination, behavior management, independent living skills training, supportive counseling, family education, discharge planning, and crisis interventions;
- Skills in interviewing, assessment, and service planning development and implementation;
- Strong ability to manage diverse duties and multiple priorities;
- Effective oral and written communication; knowledge of general principles of record keeping;
- Functions and contributes as a member of a treatment team.
Job Requirements
EXPERIENCE/EDUCATION REQUIRED:
Bachelor's Degree in a Human Services related field or equivalent education and/or experience.
Credentialed as a Qualified Mental Health Professional (QMHP) required.
