Job Description
The Community Services Worker will provide skill building and support functions for individuals with developmental disabilities to live, learn, and work successfully in their communities. This role involves providing training, skill building, and support in areas such as personal care, socialization, community integration/participation, volunteer opportunities, and daily living skills.
Key Responsibilities:
- Provide skill building and support to individuals served to promote maximum independence and facilitate participation in community and/or facility-based activities.
- Document services and supports delivered to individuals served in accordance with their person-centered plan and expressed activities of choice.
- Educate individuals on essential life skills, including feeding, personal care, and other adaptive living skills.
- Transport individuals using Agency vehicles to and from community-based activities.
Requirements:
- A minimum of one year's experience working with persons with developmental disabilities.
- A high school diploma or equivalent is required.
- A valid driver's license and copy of DMV safe driving record are also necessary.
- Candidates must be willing to complete medication aide training within five months of hire and undergo a criminal background and DSS registry check.
