Job Title: Crisis Counselor I/II/III Regional Geriatric Team (Crisis Mobile Response)
The Crisis Counselor provides support to regional state hospital discharges to nursing facilities for up to 180 days post-discharge. This position also supports local nursing homes in diverting geriatric patients from hospitalization and includes rotating 24/7 on-call coverage.

Key Responsibilities:
- Provide knowledge of behavioral health services, programs, and populations served, as well as applicable principles and practices.
- Develop relationships with nursing home staff and provide guidance on managing behavioral disorders.
- Coordinate regional hospital liaison services with Community Service Boards (CSBs) and the DBHDS Community Integration Office.
- Offer liaison and counseling services to at-risk geriatric populations throughout the region.
- Follow established discharge protocols involving barriers to discharge, transportation coordination, and initial referrals.
- Meet with individuals and their families/guardian to develop comprehensive discharge plans.
- Maintain recordkeeping through electronic health records in accordance with regulatory standards.
Qualifications:
- Knowledge of Region 3 services, programs, service providers, and population characteristics.
- Excellent word processing, spreadsheet, and database system skills.
- Good knowledge of behavioral health interventions and strategies.
- Ability to work independently and effectively as a team member.
- Effective time management and prioritization skills.
- Ability to communicate effectively with various individuals.
Minimum Requirements:
- Counselor I: Bachelor's degree or clinical Master's program enrollment in human services or healthcare administration.
- Counselor II: Master's degree with undergraduate degree in human services or healthcare administration.
- Counselor III: Licensed Mental Health Professional with experience working with geriatric or mental health populations.
