Key Responsibilities:
- Provide personal care and support to individuals served, including assistance with feeding, bathing, and other daily living tasks.
- Transport individuals using Agency vehicles to and from community-based activities.
- Document services and supports delivered to individuals served in accordance with their person-centered plan and expressed activities of choice.
- Collaborate with colleagues to ensure seamless delivery of services and supports.
Requirements:
- A minimum of one year's experience working with persons with developmental disabilities.
- A high school diploma or equivalent is required.
- A valid driver's license and copy of DMV safe driving record are also necessary.
- Candidates must be willing to complete medication aide training within five months of hire and undergo a criminal background and DSS registry check.
