Job Description

Job Title : Social Worker - Social Work - Mount Sinai Morningside - Part Time - Day

  • Assesses and evaluates patients / families' social, psychosocial and environmental situations to determine patient needs.
  • Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.

Responsibilities

  • Develop options and treatment plans in a timely manner.
  • Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
  • Ability to identify, select, access and make referrals to appropriate community resources.
  • Monitor, coordinate, manage activities for high risk psychosocial patients to ensure implementation of multiple services.
  • Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
  • Identify and assist team in managing obstacles to treatment and / or discharge plans.
  • Provide psycho-education for patients and families.
  • Knowledge of child protection, elder abuse, domestic violence policies and protocols.
  • Participate interdisciplinary rounds, case conferences, staff meetings.
  • Document in the medical record in accordance with standards set by Department of Social Work and / or regulatory agencies.
  • May supervise social work interns.
  • Qualifications

  • Masters in Social Work from an accredited school of social work. Knowledge of social work services, theories, techniques, community resources and agencies.
  • Related experience in the social service and / or health care field, preferably one year in a medical setting, which can include related field placement.
  • New York State Department of Education Licensure Licensed Masters of Social Work (LMSW).
