Job Title: Care Coordinator (Benefits and Entitlements)

 

The Care Coordinator in this position at REAP [Resource, Entitlement, Advocacy Program] will be based at 1403-05 Madison Avenue, Manhattan. REAP plays a critical role in efforts to improve the health of our growing patient population. REAP helps reduce unnecessary admissions and emergency room visits by providing advocacy and assistance around benefits procurement and maintenance and by connecting patients to community resources. 

 

The Care Coordinator joins a staff of 13 including Care Coordinators, Social Workers, Administrative Assistants, and Director. Care Coordinators will be responsible for handling a caseload of patients for whom they provide the following assistance:

  • Comprehensive screening to assess eligibility for a range of government benefit programs
  • Educating patients about the programs for which they are eligible
  • Assisting patients in applying for, securing and maintaining active coverage for eligible benefit programs
  • Collaborating with social workers and other internal and external agency staff to advocate on patient’s behalf to overcome barriers to accessing needed care

 

REAP assists with a range of government programs including Medicaid, Medicare, Child Health Plus, Essential Plan, SNAP & others. In addition to on-the-job training, PEAK modules & online webinars, Care Coordinators will attend trainings at Medicaid & will attend a 3 day training to become a Certified Application Counselor with the New York State of Health Marketplace. 

 


Responsibilities

Care Coordinators meet with patients during scheduled appointments in accordance with COVID safety guidelines. They also contact relevant public benefit programs and community agencies as needed to address patient barriers to care. They are responsible for documenting their work in the REAP Salesforce database and when appropriate in a shared database with the Finance Department.  They will have access to the electronic medical record as needed for the purpose of checking for insurance information.

 

Care Coordinators are assigned patient referrals and complete a comprehensive intake for the purpose of screening and appointment scheduling. The Care Coordinator is responsible for documenting their work in the REAP database and continuing to work with the patient over time as needed. Care Coordinators collaborate with hospital staff who refer patients to REAP and are responsible for updating the referring staff person on the outcome of their work with patients.

 

REAP utilizes a team approach and Care Coordinators work closely with other REAP staff. Teamwork is an essential part of the work at REAP as staff communicate frequently throughout the day about patient and program needs.

This position requires the willingness and the ability to be flexible as REAP explores options for responding to the growing demand for its services in a rapidly changing health care environment.


Qualifications
  • Bachelor's degree.
  • Experience in healthcare or related field is preferred.
  • Experience with government benefit programs is preferred.
  • Excellent oral and written communication skills and ability to foster a supportive environment for patients.
  • Bilingual (English/Spanish).
  • Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
  • Excellent organization and time management skills as well as attention to and accuracy with details
  • Must work well as part of a small team

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661 - $71974.47 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
