Position Title :

Community Health Advocate-Aids Non Nursing-Harlem Health Center-Full Time-Days

Position Summary :

Performs intake and assessment, service plan development, individual and group education, individual and group counseling, timely documentation, and appropriate referrals.

Responsibilities

  • Provides direct service to patients selecting service types compatible with assessment and identified adherence goals
  • Identifies, selects, accesses, and makes referrals to appropriate community resources
  • Participates in case conferences, staff meetings
  • Maintains appropriate documentation as defined by hospital policies and procedures and program requirements
  • Sets appropriate priorities and responds to follow-up situations in acceptable time period
  • Demonstrates ability to make decisions, solve clinical problems and manage resources
  • Demonstrates sensitivity to patient's cultural background
  • Understands the pertinent principles, concepts, values and relationships associated with a given task
  • Demonstrates knowledge of HIV treatment
  • Demonstrates sensitivity to patient's spiritual needs
  • Assists in data collection efforts for evaluation and research purposes
  • Retrieves CLIMACS data on patients as needed

Qualifications

Education Requirements :

  • BA degree and / or four-five years professional experience
  • Experience Requirements :

  • Project coordination experience working in a health-related project preferred.
  • Facility with case management.
  • Bilingual (Spanish / English) preferred.
  • Experience in HIV / AIDS preferred
  • Collective bargaining unit : SEIU 1199-MSSL

    Compensation Statement

    The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $24.93 - $27.09 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses / incentive, differential pay or other forms of compensation or benefits.
