Position Title :
Community Health Advocate-Aids Non Nursing-Harlem Health Center-Full Time-Days
Position Summary :
Performs intake and assessment, service plan development, individual and group education, individual and group counseling, timely documentation, and appropriate referrals.
Responsibilities
- Provides direct service to patients selecting service types compatible with assessment and identified adherence goals
- Identifies, selects, accesses, and makes referrals to appropriate community resources
- Participates in case conferences, staff meetings
- Maintains appropriate documentation as defined by hospital policies and procedures and program requirements
- Sets appropriate priorities and responds to follow-up situations in acceptable time period
- Demonstrates ability to make decisions, solve clinical problems and manage resources
- Demonstrates sensitivity to patient's cultural background
- Understands the pertinent principles, concepts, values and relationships associated with a given task
- Demonstrates knowledge of HIV treatment
- Demonstrates sensitivity to patient's spiritual needs
- Assists in data collection efforts for evaluation and research purposes
- Retrieves CLIMACS data on patients as needed

Compensation Statement
The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $24.93 - $27.09 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses / incentive, differential pay or other forms of compensation or benefits.
