Job Responsibilities Include:
- Provide kind and compassionate care for our residents with an emphasis on patient centered care, quality, and timeliness.
- Ensuring quality and regulatory compliance while overseeing coordination of residents’ health and wellness needs
- Active coordination with the interdisciplinary team in both the care planning and care delivery process.
Knowledge/Skill requirements:
- Demonstrated knowledge of clinical skills, nursing practices and techniques as applied to skilled nursing care.
- Demonstrated knowledge of federal, state and local long-term care regulations.
- Demonstrated understanding of the resident assessment and care planning process.
- Organizational and time management skills.
- Ability to effectively communicate with residents, family members and staff at all levels of the organization.
- A compassionate commitment to customer service and enjoy caring for the elderly.'
- Prior long term care experience is preferred
- Computer proficiency and electronic documentation experience preferred
- Familiarity with culture change and/or person-centered care would be a plus
- Excellent ability to foster a team-oriented work environment
License/Certification requirements:
- Graduate of an accredited LPN or RN school of nursing.
- Valid, unencumbered LPN or RN license in the state of Ohio.
- Current CPR certification.
