We have an opening for a special person to oversee Guide Dementia Program while working closely with the care management team. The MWVADC is one of only 3 healthcare organizations in New Hampshire to have been chosen by the Centers for Medicare & Medicaid Services to offer this exciting program. The Guide program is a comprehensive dementia care program funded by Medicare to improve the quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. The position is responsible for implementing, evaluating, and coordinating all local components of the national dementia program for the Adult Day Center for a geriatric population of individuals with dementia and their caregivers. This position includes both administrative duties and occasional travel for initial one-time home visits.
The successful candidate will have proven organizational, presentation and interpersonal skills in a relevant previous position. Prior experience with program development and implementation or proven accomplishments in a related therapeutic position is essential.
