What are the responsibilities and job description for the Clinical Pharmacist position at Mountain Community Health Partnership?
Mountain Community Health Partnership (MCHP) is seeking a full time Clinical Pharmacist. The Pharmacist will provide leadership and pharmaceutical excellence through direct patient care activities, as a member of a multidisciplinary healthcare team. The Pharmacist also functions in a consulting role and co-visit model with providers in a primary care setting. Consulting includes addressing medication access barriers, drug information questions, and managing high-risk patients.
Other duties of the Clinical Pharmacist include but are not limited to chronic disease management via collaborative practice agreement and education for health conditions (i.e. diabetes, asthma, hypertension, heart failure, and cardiovascular disease), interpreting and evaluating appropriateness of patient medications, assist with management of 340B program, administer and monitor log acting injectables (i.e. Sublocade, Brixadi, antipsychotic, etc.), and other duties as assigned. Clinical Pharmacist must be a Doctor of Pharmacy (PharmD) and eligible for North Carolina Clinical Pharmacist Practitioner (CPP). A PGY1/PGY2 residency and/or 1-2 years' experience in ambulatory care is preferred. An active North Carolina license or eligible to become licensed within 60 days, required.

Salary : $98,000 - $120,000
