What are the responsibilities and job description for the Community Health Worker position at Mountain Community Health Partnership?
Mountain Community Health Partnership is seeking a full time Community Health Worker (CHW). CHWs will empower patients and community members to have self-efficacy by giving them tools to manage their own health. Empower chronic disease self-management especially within high-risk, high-utilization patient population. Increase engagement with vulnerable populations, and with the community organizations that assist our most vulnerable patients. CHW position will require occasional weekend and after/before hours work due to the scope of the job. The ideal candidate will be proactive, independent, and motivated to advocate for underserved populations and have already built trust in the communities or has a track record of gaining the trust of similar communities.
Education & Experience: High School Diploma or Equivalent, Valid NC Driver's License, Reliable Transportation and Proof of Vehicle Insurance. Two years of experience in the CHW field, health education, health care, social services or related field preferred. Bilingual preferred.
