What are the responsibilities and job description for the RN Care Manager position at Mountain Community Health Partnership?
Mountain Community Health Partnership (MCHP) is seeking a full time Registered Nurse (RN) Care Manager. The RN Care Manager will address the needs of our population by assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required. This role involves working within the RN scope of practice and collaborating with the primary care provider, patient, caregivers, family members, and other members of the care management team, to coordinate a full continuum of health care services.
Key Responsibilities:
- Provide effective care management services based on case management standards of practice.
- Complete comprehensive assessments considering medical, biopsychosocial, behavioral, spiritual, and cultural needs.
- Identify and address barriers that impede health outcomes.
- Provide education to patients/family about clinical diagnosis, medications, available resources, prevention and risk factors.
- Monitor quality and effectiveness of interventions by setting patient centered goals.
Qualifications:
- Education: Bachelor's degree from an accredited school of nursing with a license to practice in NC. Certified Case Manager (CCM) preferred.
- Experience: Minimum of five years' experience in ambulatory/care management or acute care setting, home health, or public health.
- Proficient in Microsoft Office, excellent verbal and written communication skills, organized, ability to problem-solve, critical thinking skills, ability to maintain confidentiality, and interact with individuals of all cultures.
