What are the responsibilities and job description for the Community Support Associate position at Mountain Comprehensive Care Center, Inc.?
PRINCIPLE RESPONSIBILITIES:
1. Train, coach and support clients or their caregivers in the accomplishment of daily living skills such as housekeeping, cooking, personal grooming, accessing transportation, budgeting, paying bills and maintaining an independent residence.
2. Assist clients with accessing financial, housing and other public services, and in emergency situations.
3. Assistclients in meeting their needs for food, shelter and clothing.
4. Assist clients in accessing and utilizing a variety of community agencies and resources to provide ongoing social, educational, vocational and recreational support and activities.
5. Accompany clients to activities in the community.
6. Follow up with clients regarding health and service appointments, completion of forms, returning forms or receipts and other similar activities
7. Participate in Treatment Team meetings and assist clients in developing their goals and objectives for their Person Centered Plan.
8. Participate in monitoring client and family satisfaction with supports, services and programs.
9. Maintain a working knowledge of current trends and developments in the field.
10. Serve as a point of contact for crisis situations as needed.
11. Ensure compliance with Department for Behavioral Health, Developmental and Intellectual Disabilities and Agency policies and procedures.
12. Maintain and submit all required records, documents and reports on a timely basis.
13. Create and maintain files for each client on caseload.
14. All other duties as assigned.
MINIMUM QUALIFICATIONS: A community support associate shall be eighteen (18) years of age or older; have a high school diploma, or general equivalency diploma (GED), or qualifying documentation from a comparable educational entity; have one (1) year of full-time experience working with individuals who receive services for treatment of a 1) Behavioral health disorder or 2) co-occurring disorder and successfully complete within six (6) months of hire the department-led or approved training program described in Section 3 of 908 KAR 2:250. Must have a valid Kentucky driver’s license.
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of and personal experience with public mental health services. Strong reading comprehension, verbal and written communication skills. Demonstrates the ability to communicate with others in a professional manner.
