Overview
The LPN Clinical Manager Assistant is a Licensed Practical Nurse responsible for assisting with the agency’s day-to-day clinical functions. The LPN Clinical Manager Assistant works under the directive of the RN Clinical Manager/Director of Nursing. This position is responsible for assisting with clinical and outcome process workflows to ensure all services are maintained and delivered in an efficient manner according to licensure, policy, and procedure.
- Works with intake and agency with coordinating patient referrals and admissions
- Ensures initial orders received and Face-to-Face encounters are accurate and meet regulations
- Works closely with scheduler and agency to ensure timely initiation of care
- Assists in the coordination of interdisciplinary team services, visit and on call assignments, patient care, and referrals. Reviews productivity and ensures maintained by all clinicians
- Review and process incoming faxes; transcribe orders as needed per licensure (i.e., wound care orders) and notify scheduler and staff of changes
- Reviews EMR Field Chart dashboard and pending QA and assists to ensure consistent process workflow with coordination to shared services
- Monitoring pending submission folder to ensure timely submission of visits/charts
- Reviews and approves all routine visit notes and sub-forms for approval
- Reviews wound assessment to ensure all wound care protocols are met (i.e., wound pics, measurements, progress, etc.)
- Review lab orders, ensure labs scheduled, review results and sent to MD in timely manner
- Monitors transfer and HOLD patients and ensures transfers and ROC’s completed timely
- Assists in the coordination of Agency’s services with the services of other community agencies
- Monitors supplies and equipment for inventory, integrity, and appropriate use
- Participates in medical record audit
- Assist with QAPI audits
- Reviews occurrence, infection, complaint reports collaborating with Administrator/DON
- Assist with staff meetings, case conferences and in services as scheduled and maintains documentation
- Assist in recruiting, hiring, an assist with an onboarding schedule and ensure new hire orientation is being processed
- Available to share on-call and to perform nursing functions in times of staff shortage
- Graduate of a state approved school of practical / (vocational nursing
- Currently licensed in the state in which practicing
- Minimum of five (5) years’ experience in nursing
- Minimum of one (1) year of home health preferred
- Knowledge of Medicare regulations related to documentation and clinical outcomes management for home health
- Familiar with ICD-10 codes preferred
- Ability to exercise initiative and independent judgment
- Proficient in organization, planning and execution to ensure quality and productivity in work effort
- Strong interpersonal skills and ability to interact in a positive fashion with physicians, clinical team members and other company employees
- Basic computer skills and willing to learn new software programs, experience with Electronic Medical Records and Microsoft Outlook preferred
