- work directly with individuals who have intellectual or developmental disabilities. Day-to-day they assist individuals with various daily living activities like bathing, eating, administering medications, hygiene, and grooming.
- DUTIES / RESPONSIBILITIES : include but are not limited to the following.
- Participate actively in the planning and provision of all support services for individuals served by Mountain Lake Services, including day habilitation, residential habilitation, at home residential habilitation, job coaching and respite. Ensure corporate compliance and accurate documentation of services provided.
- Maintain daily and monthly progress notes in accordance with service requirements.
- Support people through advocacy, individualization, and offering choices in a manner that support their personal outcomes.
- Encourage active involvement by people in all aspects of their daily lives, with supports and training to foster skills for independence. Include the individual in decisions that pertain to her / him. Share relevant information to coordinate services and supports.
- Provide guidance, emotional support and crisis intervention as needed.
- Play a lead role in developing meaningful in-house and community activities, with an emphasis on age-appropriate activities and social experiences.
- Present an acceptable role model in areas of communication, socialization, appearance and activities of daily living.
- Utilize strategies that lead to valued social roles through community presence, participation, contribution and inclusion. Actively network within the community to help develop friendships and natural supports.
- Ensure safety, health, and hygiene in all aspects of daily life, to include promoting a clean, attractive, and safe home / work environment.
- Assist individual(s) in performing the tasks necessary to ensure nutritious and balanced meals.
- Provide instruction / support in responding to emergency situations.
- Implement recommendations of clinical therapies.
- Attend in-service training sessions as required.
- Perform other related duties and activities assigned by Assistant Managers / Managers and their supervisors.
- ADDITIONAL AWAKE NIGHT DSP RESPONSIBILITIES :
- Check all areas of residence on a regular basis throughout the night.
- Walk through house with frequency as designated.)
- Assist awake residents with any concerns (i.e. illness, emotional needs).
- Complete necessary filing, paperwork and assigned household duties (i.e. laundry, deep clean walls, windows, floors, dust).
- Complete nightly checklist as outlined daily, write end of shift notes, and ensure accurate and complete communication with A.M. staff.
- Support individuals in preparing for their day and carry out any written habilitation plans.
REQUIRED SKILLS / ABILITIES :
Ability to perform approved CPR, SCIP (Strategies in Crisis Intervention Prevention) procedures, and administer medications per Agency policy and procedures.
Remain awake and alert at all times.
Occasional bending, twisting, stooping, squatting, pushing, pulling, standing, crawling, reaching, walking, driving, traveling, kneeling, and crouching.
