PROGRAM: Community Services/Home Based Supports and Services
POSITION: Community Habilitation Specialist
REPORTS TO: Community Services Manager
Pay Range: $17.00 - $23.30 per hour
Hours Available: Monday [9:30am-5:30pm] | Tuesday-Wednesday [9:30am-7:00pm] | Thursday [11:00am-7:00pm] | Friday [11:30am-4:30pm]
JOB SUMMARY:
Community Habilitation Specialists promote individualization by honoring and offering choices; encouraging people to be independent and actively involved in all aspects of daily life. Community Habilitation Specialists provide physical assistance, training, coaching and emotional support as needed. Community Habilitation Specialists are also responsible for advocating on behalf of people receiving services by sharing relevant information on an ongoing basis, actively networking within the community to help develop friendships and natural supports and maintaining a clean, attractive, safe and healthy home environment when the opportunity allows.
DUTIES/RESPONSIBILITIES: include but are not limited to the following.
Must be able to sit, stand, and/or walk for extended periods of time.
Must be able to participate in light exercise for periods of time.
Must be able to assist program participants on/off agency vehicles
EDUCATION AND EXPERIENCE:
1. EDUCATION: High School Diploma or GED required. .
2. EXPERIENCE: Previous experience working with individuals with intellectual/developmental disabilities is preferred.

DUTIES/RESPONSIBILITIES: include but are not limited to the following.
- Will provide direct support to children and adults with ID/DD living in the community.
- Promote individualization by honoring and offering choices.
- Will serve as an advocate.
- Will partner with individuals to enhance independent living skills in all aspects of their lives.
- Plan and implement a variety of recreational activities/opportunities; Play a lead role in developing meaningful in-house and community activities, with an emphasis on age-appropriate activities and social experiences.
- Will offer exploration of new experiences.
- Present as an acceptable role model in areas of communication, socialization, appearance and activities of daily living.
- Will actively network within the community to help develop friendships, natural supports, volunteer opportunities etc.
- Will develop a respectful and collaborative relationship with families/advocates.
- Will complete all necessary documentation in a timely manner.
- Will communicate with Manager and team on a regular basis.
- Will collaborate with co-workers - for coverage and also to plan meaningful activities.
- Understand and follow agency mission, vision, and credo.
- Will attend monthly staff meetings.
- Will complete all necessary trainings.
- Will perform other related duties and activities assigned by the Manager, Assistant Director or Senior Director.
- Meet qualifications for vehicle certification and poses a valid NYS driver’s license; provide transportation as needed.
- CPR & SCIP certified.
- Visual, auditory and motor skills necessary to ensure maintenance and safety standards.
- Basic computer skills, flexible, reliable, professional and able to work independently.
- Provide oversight and support to individuals of varying degrees of ID/DD.
- Verbal and written communication skills.
- Interpersonal and customer service skills.
- Organizational skills and attention to detail.
- Time management skills with a proven ability to meet deadlines.
- Strong analytical and problem-solving skills.
- Ability to function well in a high-paced and at times stressful environment.
