An Excellent Opportunity
We are looking for a dedicated and compassionate Assistant Director of Hospice Access to join our team at Mountain Valley. This role will support the Director of Access in overseeing the intake and admission process, ensuring that prospective hospice patients and their families receive timely and compassionate care.
Key Responsibilities:
- Assist in managing the Hospice Access Department, ensuring that all inquiries from patients, families, physicians, and discharge planners are handled promptly and compassionately.
- Direct the Referral Intake Specialist, ensuring efficient communication with referral sources, coordination of referral information, and effective management of departmental workflow.
- Oversee the team of Nurse Liaisons who present hospice services to patients, families, and clinical staff, clearly communicating the benefits of hospice for individuals with terminal diagnoses.
- Coordinate with Admission Nurses to complete hospice admissions, including patient assessments, documentation, and coordination of care with the interdisciplinary team.
- Act as a secondary point of contact for patients, families, and healthcare providers, providing guidance and addressing any questions or concerns about hospice services.
- Ensure compliance with hospice policies and regulations, maintaining a high standard of care throughout the admissions process.
- Assist the Director in mentoring and providing leadership to the Hospice Access Department team, fostering a compassionate and professional environment.
Qualifications:
- Experience in healthcare administration, preferably in hospice or palliative care settings.
- Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
- Excellent communication and interpersonal skills to support collaboration with patients, families, physicians, and team members.
- Understanding of hospice regulations and the admission process.
- A compassionate and patient-focused approach to care, with attention to detail in managing the admission process.
