Key Responsibilities:

  • Assist in managing the Hospice Access Department, ensuring that all inquiries from patients, families, physicians, and discharge planners are handled promptly and compassionately.
  • Direct the Referral Intake Specialist, ensuring efficient communication with referral sources, coordination of referral information, and effective management of departmental workflow.

  • Coordinate with Admission Nurses to complete hospice admissions, including patient assessments, documentation, and coordination of care with the interdisciplinary team.
  • Act as a secondary point of contact for patients, families, and healthcare providers, providing guidance and addressing any questions or concerns about hospice services.
  • Ensure compliance with hospice policies and regulations, maintaining a high standard of care throughout the admissions process.

  • Excellent communication and interpersonal skills to support collaboration with patients, families, physicians, and team members.
  • Understanding of hospice regulations and the admission process.
  • A compassionate and patient-focused approach to care, with attention to detail in managing the admission process.
