Registered Nurse Case Manager
Location : Stokes County
Schedule :
Monday - Friday, 8-hour days, with flexibility to start between 7 AM and 9 AM.
Four 10-hour days (e.g., Monday through Thursday, 8 AM - 6 PM), with flexibility to start between 7 AM and 9 AM.
Weekend Rotation : Every 8th weekend.

At Mountain Valley Hospice, we offer compassionate, personalized care to patients and their families during life’s most challenging moments. We are currently seeking a dedicated and skilled Registered Nurse (RN) to join our team of professionals, providing exceptional care in patients’ homes, skilled nursing facilities (SNFs), and assisted living facilities (ALFs). As a part of our team, you’ll collaborate with nurses, social workers, chaplains, and providers who share a deep commitment to delivering high-quality care and support. What You'll Do :
- Deliver Exceptional Care : Provide direct nursing and supportive care to terminally ill patients with empathy and professionalism.
- Collaborate and Lead : Work closely with a multidisciplinary team to ensure comprehensive patient care. Guide and support our Certified Nursing Assistants (CNAs) in delivering top-notch care.
- Monitor and Respond : Recognize and document changes in patient conditions and communicate these effectively to the care team.
