Duties
- Maintain a hospice presence in the area hospitals
- Acts as a partner in care with physicians, case managers and other facility staff.
- Educate patients and families about the hospice benefit and services.
- Provides an initial physical assessment and admission to hospice as required.
- Initiate the referral to admission process through gathering information, education, and form completion aspects of patient intake, admissions, and transition to home.
- Perform hospice in place (hospital) admissions as needed.
- Valid Registered Nurse License
- BLS Certification
- Valid Driver’s License
- Monday - Friday
- 8am - 5pm
