Job Overview
We are seeking a dedicated and compassionate Physical Therapist Assistant to join our healthcare team as Therapy Program Manager. The ideal candidate will work with and coordinate licensed physical therapists to provide high-quality care to patients in three skilled nursing facilities and a home health agency, as well as assisting with treatment of residents. This role is essential in helping patients regain mobility and improve their quality of life through tailored rehabilitation programs. This candidate will coordinate the location of the therapists, manage timesheets and payroll, and be the point of contact in Howell County.
Responsibilities
- Assist physical therapists in developing and implementing individualized treatment plans for patients in Howell County.
- Conduct therapeutic exercises and activities as directed by the supervising therapist.
- Monitor patient progress and report any changes or concerns to the supervising therapist.
- Educate patients and their families about treatment plans, exercises, and home care strategies.
- Maintain accurate documentation of patient care activities and progress notes.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Adhere to safety protocols and maintain a clean, organized work environment.
Skills
- Strong knowledge of physiology and anatomy to effectively assist in patient care.
- Experience in various settings such as skilled nursing facilities, outpatient clinics, inpatient care, home health, or geriatrics is highly desirable.
- Familiarity with discharge planning processes to facilitate smooth transitions for patients.
- Ability to work with diverse populations, including pediatrics and geriatrics, ensuring age-appropriate care strategies are utilized.
- Excellent communication skills to effectively interact with patients, families, and healthcare team members.
- Strong organizational skills and attention to detail in documenting patient progress and treatment outcomes.
