JOB SUMMARY: The Licensed Clinical Social Worker (LCSW) supports MVH's initiatives toward clinical integration, improving quality and patient satisfaction, outcomes, and reducing the total cost of care. This position requires flexibility, independence, and knowledge of the healthcare system.
The LCSW will provide assessment, intervention, and advocacy as needed to patients with varying mental health conditions, coordinating with PCPs and other care providers.

DUTIES AND RESPONSIBILITIES:
The LCSW will:
- Complete initial screenings of patients referred from primary care providers
- Employ interventions such as care coordination/case management, risk screening, and behavior change
- Offer solutions to roadblocks while implementing programs
- Monitor high-risk patients within each clinic and follow up with clinic staff
- Ensure deadlines are met during implementation stages
- Revised care plans as indicated by patient response to treatment and provider direction
- Perform patient care responsibilities considering needs specific to the standard of care for patients
- Be knowledgeable of medications and their correct administration based on age and clinical condition
- Formulate a teaching plan based on identified patient learning needs
- Advocate on behalf of patients and their families to gain access to services and resources
- Provide provider support in complex cases
- Treat patients and their families with respect and dignity
- Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families
- Function as a liaison between administration, patients, physicians, and other healthcare providers
- Interact professionally with patient/family and involve patient/family in the formation of the plan of care
- Demonstrate knowledge of principles of growth and development over the lifespan
- Communicate appropriately and clearly to manager, co-workers, and physicians
- Maintain good working relationships within the department and with other departments
- Act as a resource for other less experienced staff
- Document meets current standards and policies
- Coordinate and supervise patient care
- Complete annual education requirements and keep licensure current
- Maintain patient confidentiality
- Represent the organization in a positive and professional manner
- Comply with organizational policies regarding ethical business practices
Qualifications:
The LCSW should have:
- High School graduate or equivalent
- Current BLS certification, including training on defibrillators and renewal as required
- Proficient in MS Office and Excel
- Ability to learn complex phone systems and hospital information system
- Excellent listening and communication skills in English
- Even-tempered and able to make sound decisions and judgments
- Ability to prioritize multiple and competing customer and staff needs
- Excellent interpersonal skills
