- Develop and implement personalized physical therapy treatment plans for residents.
- Assist with the new sensory program, focusing on resident engagement and sensory stimulation.
- Collaborate with interdisciplinary teams to ensure comprehensive resident care.
- Conduct evaluations to determine therapy needs and document progress.
- Educate residents and their families on at-home exercises and wellness strategies.
- Maintain accurate records and comply with all facility policies and procedures. Qualifications :
- Current state licensure as a Physical Therapist.
- Strong communication and collaboration skills.
- Passion for working with residents in a post-acute care setting.
