We are looking for a dynamic and passionate Social Worker (MSW) to join the Mountainside Team!
What You'll Do
The Social Worker (MSW) will be responsible for providing direct clinical social work services to patients and their families in a multidisciplinary team approach
The successful candidate will be able to :
- Completes an initial psycho-social assessment including social work treatment plan
- Performs any other duties and assignments as assigned
- Provides individual, family or group counseling, complex discharge planning, bio-ethics case consultation, referrals to community agencies and entitlement programs
- Ensures continuity of care for those patients with complex post-acute care needs and maximizes the patient's and family's abilities to meet the continuing care needs within their social and financial framework
- Understands stages of growth and development to assess each patient's age specific needs to provide age specific treatment and care
- Documents interventions in patient's chart and submits monthly statistical information to the department
- Performs any other duties and assignments as assigned
