Summary: The Licensed Practical Nurse (LPN) provides services in accordance with agency policy. Prepares clinical notes, assists the physician and the registered nurse, prepares equipment and materials for treatment and assists the client in learning appropriate self-care techniques. The LPN provides nursing care to assigned clients under the direction of a Nurse Case Manager. Performs nursing care as defined within the Nurse Practice Act and Mozaic Senior Life Home Care Agency policy and procedure.
Reports To: Nurse Case Manager
Essential Responsibilities:
- Provides direct nursing care, personal care, treatment, and administration of medications to assigned patients under the direction of a Registered Nurse.
- Complies with infection control standard precautions and safety instructions.
- Documents in the clinical record accurately, clearly and concisely, and follows agency policy for timely submission.
- Reports changes in patient needs and physical / mental status to the Registered Nurse Case Manager.
- Provides accurate information to health care professionals involved in the total care of the client.
- Establishes and maintains effective interpersonal relationships with clients and significant others, nursing staff and other disciplines.
- May orient home health aide to plan of care create by the RN, may not perform supervision of aides.
- Performs technical skills safely and according to accepted procedures.
- Maintains CPR certification and demonstrates the abilities to initiate emergency measures.
- Keeps current with CEUs per states regulation.
- Follows established lines of communication and authority.
- Complies with attendance policy and dress code.
- Other duties as assigned by the Registered Nurse.
Minimum Qualifications
Education Graduate from an accredited school of nursing. Current valid LPN license with the State Board of Nursing. Must meet all continuing education hours and education requirements. Ability to read, write and communicate in English.
- Experience: Minimum of six months' work experience as an LPN or previous experience as a CNA in home health.
- Experience in home health or related health care field is preferred.
- Licensure: LPN, State of Connecticut, CPR certification
