(MPAC) Monumental Post Acute Care at Woodside Park is seeking Certified Nursing Assistants to join our team in Philadelphia, PA. Full-Time, Part-Time, and PRN Positions are available.
The Licensed Practical Nurse assesses, plans, delivers, and evaluates nursing care to a specific group of patients. May be designated as Charge Nurse for any given tour of duty and is then responsible for supervision of partial or total nursing services on his/her assigned unit.
- Assumes accountability and responsibility for completion and quality of work.
- Implements the Nursing Process by resident interview, observation, examination, and review of significant records.
- Contributes to the development and evaluation of a goal-orientated care plan continuously based on nursing assessments and nursing interventions with the participation of patients and significant others.
- Implements and delegates nursing interventions that are consistent with the care plan and the medical regime.
- Documents all resident responses including behavior changes, physiological changes, nursing interventions to achieve goals, prescribed medical regime, and nursing interventions to achieve a desired resident response.
- Attends mandatory programs to meet state, federal, and Bala Nursing and Rehabilitation Center's requirements and assigns staff to attend.
- Accepts individual responsibility for program attendance by arriving in a timely fashion, maintains records by printing name and signature on permanent records, and mandatory personal attendance records required for continual employment.
- Assures discharge planning is consistent with the patient's status and reflects collaboration with interdisciplinary team members.
