Care Manager LVN, LPN, LSW (Waco Only)

Day-to-day responsibilities of this the role and a description of the project:

• SC will have 3 days of visits and 2 days of documentation weekly
• SC will have a caseload and work assigned tasks
• SC will complete H2060 assessment
• SC will fill put required forms for each assessment
• SC will make follow up calls

Performance and Expectations / Metrics for this individual and their team:

• SC will complete documentation within 3 day turnaround time
• SC will stay organized and ensure work they do not fall behind
• SC will maintain a 92% or high in Audit score

What previous job titles or background work will in this role:

• LVN, Service Coordinator, Care Manager, Case Manager, Social Worker

Skills/ Experience:

• 2 years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
• Service Coordination, Case Management or Nursing experience preferred.
