Interim : Transitions of Care (TOC) Nurse
Telecommuting opportunity, Remote opportunity for
California Residents.
Join a California Healthcare organization looking for an Interim Transitions of Care (TOC) Nurse to conduct telephonic remote concurrent review of inpatient activities. Additionally, the TOC Nurse is responsible for ensuring that member care is rendered at the most appropriate level, for the requisite length of time, in compliance with the organization’s regulatory and contractual requirements. Makes timely and appropriate recommendations regarding transfers, determination of level of care and discharge planning. Escalates non-standard requests to Medical Director for review; approves or denies services as directed. Tracks data as required; compiles data for, produces and submits routine reports. The Interim Transitions of Care (TOC) Nurse reports to the TOC Nurse Manager.
Interim details :
Must be a current resident of California, required.
- 3–4-month Interim assignment position to begin with. Position could go permanent, but there is NO guarantee that this will occur.
- Able to work remotely from California, required.
- Basic knowledge of computers required.
Compensation : $55.00 Per / Hr.
