What We’re Looking For
We are seeking a licensed and experienced Practical Nurse (LPN). There are a few opportunities in several areas, including TN, PA, MD, VA, and NC.
Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. He/she operates within the scope of practice defined by the State Nurse Practice Act. The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided by unlicensed staff.
What You’ll Be Doing
Patient Evaluation: Collects, reports and documents objective and subjective data and observes conditions and reports changes in condition to RN
Care Planning: Contributes to establishing individualized patient goals; Assists in developing interventions to achieve goals; Implements the plan of care; and Evaluates effectiveness of interventions to achieve patient goals and minimize rehospitalizations; Participates in review and revision of plan of care; and Provision of Direct Patient Care: Administers medications and performs treatments per physician orders; Communicates pertinent data to RN and/or physician; Documents accurately and thoroughly; and Consults and seeks guidance from the RN as necessary.
Care Management: Facilitate appropriate discharge planning: Communicates team’s recommendations for discharge to the attending physician, patient and family; Prepares patient/family for discharge including patient and family education; and Monitors patient care provided by unlicensed staff.

Your Qualifications
- Graduate of an approved school for practical or vocational nursing is required;
- Must have a valid Registered Nurse License or LPN License in the state(s) of practice, or proof of license eligibility; and must be currently licensed by the State Board of Nursing
- Experience is preferred in rehabilitative or geriatric nursing
- Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy. TSTs will be administered at the work site if required.
- Maintains current BLS/CPR certification (All SNFs and Behavioral Centers; ALFs/ILFs as required by State).
- Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence
- Must be able to meet the physical demands on the position; including lifting 50 lbs, standing or walking for long hours, and other activities.
- Must comply with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
- You must be able to meet all credentialing and licensing verification that is a requirement prior to the interview.
