Job Description
The basic functions of the Housing Specialist are to support homeless individuals, in the recovery process and engage with them in a consistent basis to secure safe and affordable housing.
What you’ll do:
- You will conduct assessments and interviews with clients to determine eligibility for the program. You will develop and update action plans, case notes, and other documents related to a client’s file. You will Provide ongoing life skills development to assist the client in improving independence and quality of life. You will support clients by guiding through the process of application, qualification for SSI, SSDI, Medicaid and Medicare, SNAP, and other services as needed.
- You will connect clients to medical, mental health, and other supportive counseling services. You will provide specialized and targeted interventions when clients are at risk of eviction. You will advocate for clients. You will build partnership with local landlords to secure housing for client. You will occasionally conduct visits in the client’s home to ensure compliance with lease. You will support the client by ensuring they understand their rights and responsibilities as tenants.
- You will Track and report outcomes and provide data updates as directed. You will attend and actively participate in scheduled staff meetings. You will Attend external and internal case staffing meetings as necessary to coordinate services
Job Type: Full-time
Pay: $17.50 - $20.00 per hour
