Licensed Clinical Social Worker (LCSW) | Remote in Indiana | Full Time
We are seeking a compassionate and experienced Licensed Clinical Social Worker to join a mission-driven organization focused on delivering person-centered care to individuals and families across Indiana. This is a remote role with the flexibility to make a lasting impact through direct support, resource navigation, and care coordination, all while working in close collaboration with an interdisciplinary team.
This position plays a key role in identifying and addressing the social determinants of health (SDOH) that influence patient well-being. The ideal candidate brings a strong understanding of social work principles, excellent interpersonal skills, and a desire to help people in underserved and vulnerable communities thrive.
What You’ll Do
- Conduct psychosocial assessments to identify patient needs and barriers to care.
- Provide short-term counseling, crisis intervention, and referrals to appropriate community resources.
- Collaborate with clinical teams and community partners to develop individualized care plans.
- Support patients through transitions of care and ensure timely follow-up.
- Advocate for client needs and promote health equity through culturally sensitive care.
- Maintain accurate documentation in electronic health records and ensure compliance with professional standards and regulations.
- Participate in care team meetings and support ongoing quality improvement initiatives.
What You Bring
- Active and unrestricted LCSW license in the State of Indiana.
- Minimum of 3 years of experience in a healthcare or community-based setting preferred.
- Bachelor’s degree in Social Work required (Master’s preferred).
- Proficiency with Microsoft Office and comfort using electronic health records.
- Strong communication, organizational, and collaboration skills.
- A proactive, empathetic approach to client advocacy and resource connection.

Salary : $70,000 - $80,000
