Nurse Practitioner

We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including primary care, hospital, chronic disease, home health. Will work collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.

The Nurse Practitioner will have the following responsibilities:

• Performing comprehensive in-home assessments for identified patients of the practice and assisting the primary care team in better managing chronic diseases
• Reviewing current symptoms and identifying diagnosis to be used in care management and active medical management of treatment
• Completing physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings
• Communicating assessment findings that will be used to inform the PCP of potential gaps in care
• Accuracy on risk capture, high quality metrics and closure rates
• High patient satisfaction and improved health status including reduced hospitalization and ER visits
• Thinking critically to address patient needs, providing the appropriate resources and follow-up care to improve stability
• Educating patients on topics such as disease process, medication, and compliance
• Complying with all HIPAA regulations and maintaining security of protected health information
• Ability to travel 5 days a week to provide care to patients in their home based on the needs of business/market and/or in a clinic setting in a residential care facility.

Required Qualifications

• Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners. An active unrestricted medical license in the State of New York is required.
• At least 3 years of active practice required. DEA Certification required.
• Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner.
• Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents.
• Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.
