We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our healthcare team. Key Responsibilities:
- Perform comprehensive in-home assessments to support chronic disease management.
- Identify gaps in care and communicate findings to the Primary Care Provider (PCP).
- Ensure accuracy in risk capture, quality metrics, and closure rates.
- Educate patients on disease processes, medication adherence, and preventive care.
- Travel 5 days per week to provide care in-home or telephonically.
- Maintain HIPAA compliance and uphold the highest ethical standards.
Qualifications:
✔ Active Nurse Practitioner (NP) license in New York (unrestricted).
✔ Minimum of 3 years of clinical experience.
✔ DEA Certification required.
✔ Strong communication, interpersonal, and presentation skills.
✔ A team player with strong clinical judgment, ethics, and a passion for patient care.

Salary : $120,000 - $145,000
