JOB SUMMARY

The Advanced Registered Nurse Practitioner (ARNP) provides comprehensive, patient-centered care to elders and vulnerable adults within a tribal community, working both independently and collaboratively with physicians in a family practice setting and through home visits. The ARNP emphasizes chronic disease management, preventive care, and addressing age-related health concerns, all with a focus on improving quality of life through culturally relevant care in a home-like setting.

Major duties and responsibilities of the position:

 

1. Ability to work effectively in high-stress situations while maintaining professionalism and empathy.
2. Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice wither independently or in collaboration with PCP for patient.
3. Develop and implement an individualized plan to achieve optimal population health and disease management.
4. Order and interpret labs, tests, and diagnostic studies.
5. Prescribe pharmacologic and non-pharmacologic interventions.
6. Initiate referrals and consults to specialty services.
7. Provide education to patients, their families and caregivers.
8. Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
9. Collaborate with our Elder’s program, make home visits to patients and work as an interdisciplinary team.
10. Effectively communicate and collaborate with many disciplines to meet the needs of patients.
11. Collaborate with the in-house Adult Protective Services program focused on restorative healing.
12. Lead or assist in conflict resolution involving staff, clients, or families, using effective negotiation and mediation techniques.
13. Conduct internal investigations related to client complaints, incidents, or staff performance issues, documenting findings and recommending action steps. Ensure investigations are handled professionally, fairly, and in alignment with tribal and program policies.
14. Participate in audits, reviews, and program evaluations to promote continuous improvement.
15. Ensure program compliance with tribal, state, and federal healthcare regulations.
16. Participate in ongoing safety and quality assurance oversight within the clinic setting. Assist in the compilation of data and offers input to ensure safe working environment and provides input for standards of care.
17. Attend regular staff meetings and prepares agenda items as appropriate. Support ongoing staff and medical education process and offer clinical education to staff.
18. Provide technical supervision to Nursing staff as it relates to patient care.
19. Maintain current licensure and certifications pertinent to position. Practice within scope of licensure and seeks consultation with medical provider when appropriate.
20. Maintain the privacy and confidentiality of all protected health information in accordance with the Division’s Policies and Procedures, and in compliance with state and federal regulations.
21. Because of the Tribe's commitment to community service and the well-being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities.

EDUCATION - EXPERIENCE AND TRAINING FOR POSITION

Required

 

• Graduation from accredited program for either a Master of Science in Nursing (MSN) or a Doctor of Nursing Practice(DNP)
• Washington state license as an Advanced Registered Nurse Practitioner (family nurse practitioner), Certification as FNP by ANCC, AANP or equivalent.
• Three to five (3-5) years’ experience in primary care or family medicine and at least 1-2 years of geriatric experience required.

 

AND


• Current CPR and BLS certifications
• Valid Washington State driver’s license.

 

Preferred:


• Previous experience in a tribal health care environment preferred.
• Experience in psychiatric care, mental health services, or behavioral health settings is highly desirable.
• Expertise in conflict resolution, de-escalation techniques, and investigative processes.
• Understanding of trauma-informed care principles and culturally responsive service delivery

 

 

SPECIFIC SKILLS/KNOWLEDGE/ABILITIES REQUIRED FOR POSITION

Skills:

  • Clinical skills for hands-on evaluation and treatment.
  • Patient assessment skills, including the ability to observe and collect subjective data, make proper assessments, and develop treatment plans.
  • Effective verbal and written communication with patients and staff.
  • Time management and organizational skills for task prioritization and delegation.
  • Problem-solving and decision-making, encompassing fact-finding, research, prioritization, plan development, and evaluation of effectiveness.
  • Resourcefulness and the ability to enlist support when needed.
  • Strong written communication and documentation skills, including legibility, accuracy, and thoroughness in charting, note-taking, and requisitions.
  • Teamwork skills, demonstrated ability to work collaboratively with other providers, medical support staff, and front office staff in a professional manner.

 

Ability:

  • Ability to evaluate and treat patients with various diagnoses and medications.
  • Conduct assessments and implementing treatment plans for pressure ulcer prevention and management
  • Wound care especially for patients with diabetes or limited mobility.
  • Conduct comprehensive medical exams, including fall risk assessments, depression screening, nutritional evaluations, and Medicare wellness exams.
  • Teaching skills, including instructing clients, caregivers and other staff using appropriate learning models.
  • Strong time management and organizational skills, with the ability to prioritize multiple tasks concurrently and delegate as needed.
  • Problem-solving and decision-making, including fact-finding, research, prioritization, plan development, and effectiveness evaluation.
  • Resourcefulness and the ability to enlist support when required.
  • Competency and proficiency in using electronic medical record systems (EMR) and documentation standards.
  • Clinical skills for hands-on evaluation and treatment of symptoms and disease processes in a family practice environment.
  • Effective verbal and written communication with patients, staff, and the ability to adhere to HIPAA and other health information regulations.

 

Knowledge:

  • Knowledge of geriatric care/aging process including; distinguishing between normal cognitive decline. and dementia, falls, incontinence, delirium, pressure ulcers, malnutrition, and frailty.
  • Knowledge of chronic diseases, including diabetes, hypertension (HTN), cardiovascular disease (CVD), asthma and COPD.
  • Polypharmacy management - Evaluating medications for potential adverse effects, drug interactions, and de-prescribing where necessary.
  • Understanding of vaccines, immunizations, and physical requirements.
  • Familiarity with HIPAA and other laws/regulations related to protected health information.
  • Competence in all areas of medical practice, with a strong assessment ability to observe and obtain subjective data, make proper assessments, and develop treatment plans.
  • Knowledge of Medicare, Medicaid, and home healthcare regulations pertaining to charting requirements for reimbursement and regulatory compliance.

PHYSICAL REQUIREMENTS

Work is performed mostly in a clinic setting. Continuously uses vision and listens. Frequently sits, stands, and/or talks. Requires manual dexterity for computer work and routine paperwork. Occasionally walks, climbs stairs, bends at the waist, drives, and/or reaches with hands and arms. Occasionally lifts and/or carries up to 50 lbs. Occasionally pushes or pulls up to 250 lbs. May occasionally be exposed to moving mechanical parts and/or biohazardous waste materials.

Salary : $104,354 - $177,397
