JOB SUMMARY
The Director of Rehabilitation is responsible for ensuring that patient rehabilitative care is coordinated and managed appropriately. This position is responsible for ensuring that therapy services are delivered appropriately, documentation of therapy services meets all requirements as well as the supervision of therapy professionals.
DUTIES AND RESPSONSIBILITY
- Review and evaluate each case by reviewing the services provided by the therapists, including case conferences and documentation review, discuss and verify impressions, instruct and guide therapists to promote more effective performance and delivery of quality home health services, and is available at all times during operating hours to assist therapists.
- Assist therapists in establishing immediate and long-term rehabilitation goals, in setting priorities, and in developing plan of care.
- Assist therapists in overseeing assignment and supervision of therapy assistants.
- Attend case conference meetings with organization personnel to facilitate coordination of care.
- Conduct quarterly record reviews and communicates findings and recommendations to Director of Nursing and appropriate organization personnel.
- Complete on-site supervisory visits.
- Maintain accountability for therapy staff productivity, utilization and documentation standards.
- Assist in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
- Assist in the orientation of new therapy personnel.
- Assist Director of Nursing in the planning, implementation and evaluation of in-service and continuing education programs.
- Assist in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
JOB REQUIREMENTS
- Current CA license as a Physical Therapist.
- Two years of home health experience as a PT in the field.
- Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home health.
- Complies with accepted professional standards and practice
- Complies and maintains current CPR certification.
- The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
- Demonstrates excellent observation, verbal and written communication skills.
