JOB SUMMARY
Muir Home Health is seeking a RN Hospital Liaison to serve as a crucial link between our agency and local hospitals, facilitating smooth patient transitions from hospital to home health care. The ideal candidate will leverage clinical knowledge, communication skills, and business development experience to build and maintain strong relationships with referral sources, patients, and families. As an expert in home health services, you will educate patients and providers, assess care needs, and ensure a seamless process for admissions into our home health system.
This role requires strong clinical expertise combined with the ability to market and advocate for home health services effectively. You will work with physicians, referral sources, and the healthcare team to ensure that each patient receives optimal care, leading to positive health outcomes.
DUTIES AND RESPSONSIBILITY
- Serve as a liaison between hospitals, physicians, and Muir Home Health to promote home health services and coordinate patient transitions.
- Educate healthcare professionals, patients, and families about home health services, ensuring they understand the benefits and how to access care.
- Conduct thorough on-site patient assessments to determine the appropriateness for home health services, considering the patient’s psychosocial, physical, and economic factors.
- Develop and maintain strong relationships with referral sources, including hospitals, nursing homes, and physician offices, to drive admissions and ensure care coordination.
- Collaborate with physicians, referral coordinators, and internal teams to streamline the referral process, obtaining necessary documentation for admissions.
- Communicate and coordinate with families and caregivers, ensuring they understand and are comfortable with the home health care plan.
- Act as an educator in the community, developing relationships and building awareness about Muir Home Health’s services.
- Maintain compliance with all local, state, and federal regulations related to home health care.
- Monitor progress toward admissions, product, and payor-mix goals, documenting all interactions and referral activities in a CRM system.
JOB REQUIREMENTS
- Current CA Registered Nurse (RN) license.
- Minimum of 2 years of nursing experience, preferably in home health, hospital discharge planning, or case management.
- At least 2 years of experience in healthcare marketing, business development, or a liaison role within home health, hospice, or long-term care.
- Experience with referral and intake processes, particularly in a hospital or healthcare setting.
- Strong clinical knowledge to understand patient needs, review medical charts, and communicate with physicians regarding home health plans of care.
- Ability to establish rapport quickly and prioritize patient needs while balancing clinical, economic, and social factors.
- Must be a licensed driver with reliable transportation and meet state insurance requirements.
- Proficient with CRM systems for tracking referral sources, patient interactions, and admissions.
