Summary:
Responsiblitlies:
- Provide direct care and support tailored to individuals with medical complexities.
- Assist with daily living activities, including personal care, meal preparation, and mobility support.
- Implement care plans and follow medical protocols as directed.
- Foster a safe, respectful, and empowering environment for individuals.
- Support community involvement and engagement through outings and activities.
- Document and report on client progress, concerns, and any incidents.
- Collaborate with healthcare professionals, family members, and other support staff.
Required Qualifications/Skills/Training:
- High school diploma or equivalent (required); relevant certifications or training preferred.
- Experience working with individuals with medical complexities, disabilities, or special needs is highly valued.
- Ability to assist with personal care tasks and follow medical care plans.
- Strong communication, patience, and problem-solving skills.
- Valid driver’s license and reliable transportation.
- Ability to pass a background check and meet all state and program-specific requirements.
