Home Care LPN

MultiCulture Home Care, a Modivcare Personal Care Service, is looking for a Field Supervisory Nurse - LPN to provide quality nursing care and related services in patients' homes. You will report to the local Branch Manager.

• Pay: $30 Hourly
• Schedule: M-F 8:00AM-5:00PM
• Office Address: 354 Merrimack St, Lawrence, MA 01843

You will... 

• Provides quality nursing care and related services in the home.
• Provides those services requiring skills using the Nursing Process (Assess, Plan, Implement, and Evaluate.)
• Continually evaluates the client's nursing needs and works with the client/caregiver in developing ways to meet their needs.
• Follows a current, individualized treatment plan by making revisions based on original and ongoing assessments and in conjunction with the doctor, case manager, and client/caregiver.
• Informs the physician, case manager, and other healthcare team members of changes in the client'/s condition and needs.
• Initiates and performs teaching of the client, family, and other health team members, as appropriate, to meet identified needs and achieve goals.

• CPR Certified
• 1 year of Home Care experience preferred
• Ability to provide care in a variety of settings
• Valid Driver's License, insurance, registration, and reliable car
