Independence Healthcare, a Modivcare Personal Care Service, is looking for a RN Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers / Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.
- Schedule : M-F 8 : 30AM-5 : 00PM
- Visiting Client Territory : Worcester and surrounding areas

- Pay Range : $36-$40 / Hourly
- Sign on Bonus : 5K

Performs on-site supervisory and teaching visits to evaluate caregiversProvides clinical orientation to caregivers on assigned home care cases.Participates in the training of new nurses and in-services for field staff.Performs initial nursing assessmentsCounsels patient / caregiver in meeting nursing and related needs.Assigns and coordinates services provided.Assists physician in developing a plan of treatment / care and makes necessary revisions based upon patient's condition / needs.Prepares clinical progress notes ensuring any changes in the patient's condition is reportedMonitors clinical records for complete and accurate documentation.We are excited to speak to someone with the following
