We are seeking a dynamic and dedicated Licensed Behavioral Health Provider to join the team with Multnomah County Health Department's Mobile Clinic. This is an exciting opportunity to work directly with houseless and other economically vulnerable Multnomah County residents who utilize our dynamic Mobile Clinic program for drop-in primary care.
The Mobile Medical Van program is a part of Multnomah County's primary care division, called Integrated Clinical Services (ICS). It is in fact the largest Federally Qualified Health Center (FQHC) in Multnomah County, providing quality health services for people who experience barriers to accessing care. Our vision is to provide services that are integrated and connected to support optimal health, reducing wait times, and optimizing access.
This nationally innovative mobile primary care clinic offers medical, dental, lab and now integrated behavioral health services at rotating locations around Multnomah County. The Mobile Medical Van Behavioral Health Provider will be an integral part of the primary care team delivering high quality, compassionate and trauma informed behavioral interventions to patients of this unique clinic.
What You'll Do :
As the Behavioral Health Provider, other duties include :

- Personalized Care Architect : Tailor care plans to each patient's unique needs and preferences, collaborating closely with primary care teams to deliver comprehensive, patient-centered support.
- Mind-Body Connection Expert : Leverage your deep understanding of the interplay between physical and mental health to optimize treatment plans and patient outcomes.
- Flexible Care Innovator : Embrace a dynamic hybrid model of care, seamlessly blending in-person visits with telehealth / telemedicine to provide accessible and convenient support to patients.
