- Care Manager, RN – USC Only

- New Castle County, DE (must reside in Delaware)
- Key Responsibilities :
- Conduct face-to-face needs assessments with members in homes, hospitals, motels, and other community settings.
- Plan, coordinate, and evaluate care for members with chronic and complex health needs.
- Facilitate transitions between care settings (e.g., hospital to home, nursing facility to community).
- Educate members and caregivers about healthcare, resources, and services.
- Develop and implement care plans in collaboration with members and their healthcare team.
- Ensure effective service delivery, evaluate care plans, and make revisions as necessary.
- Maintain accurate documentation and comply with privacy practices and regulations.
- Required Qualifications :
- Registered Nurse (RN) in Delaware with case management experience.
- Experience with assessments, care / service planning, and collaboration with healthcare providers.
- Experience with DME equipment orders, discharge planning, and social determinants of health.
- Proficiency in PC-based word processing, Excel, and Outlook.
- Reliable transportation for travel within assigned area.
- Flexible hours to meet member needs.
- Knowledge of cultural competency and the ability to work with diverse populations.
- Experience in electronic documentation systems.
- Organized and able to meet regulatory deadlines.
- Preferred Qualifications :
- Certified Case Manager (CCM).
- Social Work Licenses (LBSW, LMSW, LCSW).
- Experience with HIV / AIDS, behavioral health, and developmental disabilities populations.
- Medicare and Medicaid experience.
