Job Title : Housing Coordinator
Location : Hybrid position – remote work but requires field work at least once per week, and as needed. Must be located around Lehigh Gap , PA or Pittsburgh, PA
Job Description :
The Community Housing Coordination program assists Medicaid members in navigating local, state, and regional housing agencies and services to obtain or keep safe, stable, accessible, and affordable housing.
Job Responsibilities :
- Directly engages with members to connect them to the resources that can assist in resolving their housing-related needs.
- This role is responsible for developing and maintaining a referral network of housing resources specializing in advocacy and direct assistance for diverse needs.
- The goal of the program is to support the members’ independence and well-being while meeting their expressed housing-related needs where able through established and novel community-based partnerships and direct engagement.
