Evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software
Additional Job Description
Bachelor degree in Nursing from an accredited school of nursing and five years of work experience as a registered nurse required. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
