Job Description
The Home Health Social Worker provides psychosocial assessments, diagnosis and treatment to patients in their place of residences and prescribed by a physician, to assist the patient / family member(s) / caregiver(s) develop effective coping skills in response to the patient's diagnosis, treatment and / or life situation.
Entity / Organization : MUHA (Medical University Hospital Authority / Medical Center)
Hours per week : 40 hrs. weekly
Scheduled Work Hours / Shift : Monday - Friday, Days
Fair Labor Standards Act Status : Hourly
Job Summary / Purpose : The Home Health Social Worker provides psychosocial assessments, diagnosis and treatment to patients in their place of residences and prescribed by a physician, to assist the patient / family member(s) / caregiver(s) develop effective coping skills in response to the patient's diagnosis, treatment and / or life situation.
Minimum Training and Education : Master's degree in a social work or social welfare program accredited by the Council on Social Work Education and three years professional social work experience.
Required Licensure, Certifications, Registrations : MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
