What are the responsibilities and job description for the Home Health Nurse, LVN position at Music Home Health?
Responsibilities:
- Provide direct patient care, plan of treatment implementation, and evaluation of patients' conditions
- Monitor and document patients' vital signs and medical history
- Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans
- Educate patients and their families on disease management, treatment options, and preventive measures
- Perform catheter and ostomy care if necessary - Ensure proper sterilization of medical equipment through sterile processing techniques
- Assist with suturing wounds and providing wound care
Requirements:
- Valid LVN license in the state of California
- Minimum of 1 year of nursing experience in a clinical setting
- Proficiency in using Synergy or similar electronic medical record (EMR) systems
- Knowledge of Durable Medical Equipment (DME) and its usage
- Familiarity with special needs patient care and accommodations
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
