CES Outreach Navigator

GENERAL STATEMENT OF DUTIES
The CES Outreach Navigator works with regional and local social service agencies to ensure that homeless persons and those at risk of homelessness are structurally assessed for services, assigned to an appropriate program or service for intervention, and assisted in process of engagement until persons are stably housed. The CES Outreach Navigator works with MUTEH and the CoC to conduct street outreach and community agency outreach.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Reports directly to the Program Director.
- Working knowledge of programs and processes with CoC CES including SSVF, rapid resolution, outreach, housing, and case management.
- Engage new regional partners and providers to further expand MS BOS COC's collaborative network.
- Provide support for newly trained providers on the Coordinated Entry System, including data collection and entry.
- Assist in implementing regional coalition meetings.
- Participate in monthly regional outreach teams and outreach efforts in the BoS communities.
- Monitor regional lists of homeless individuals and families. This includes working with outreach teams, housing providers, and community agencies to ensure clients have moved through the Coordinated Entry System appropriately and our prioritization policies are being followed.
- Refer waitlist clients to housing agencies and conduct weekly/bi-weekly follow-up calls.
- Review agencies' service progress through weekly data quality reports, Monthly Progress Reports, and quarterly report cards.
- Receive and communicate referrals to regional outreach teams and agencies.
- Ability to coordinate and lead outreach activities in coverage areas.
- Conduct screenings with clients to determine program eligibility.
- Collect appropriate eligibility documentation on program applicants.
- Collaborate and seek to develop new relationships with representatives in the community and other agencies to increase engagement with homeless individuals.
- Document all engagements with applicants in HMIS Street Outreach module.
- Track encampment locations using GPS technology.
- Act as a liaison between client and community agencies as needed.
- Participate in weekly supervisory and/or quarterly staff meetings, training and development opportunities as determined appropriate by the Program Director.
- Participate in resource connections to strengthen the referral services of the CoC network.
- Other duties as assigned.
QUALIFICATIONS AND KEY COMPETENCIES
- A Bachelor's Degree in Social Sciences or a related field from an accredited school is preferred, or a minimum of two (2) years of experience in social services accompanied by an Associate's Degree.
- Must be proficient in computer applications including Microsoft Office and knowledge in data entry and database reporting.
- Proven ability to work independently and as an effective and collaborative member of a team.
- Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.
- Other Core Competencies expected: time management, attention to detail, customer service, interpersonal skills, caring, reliability, collaboration, initiative, fostering diversity, and organizational understanding.
OTHER REQUIREMENTS
- Continuously exchanges information through listening and talking with clients, agency staff, employers, representatives of community organizations and other individuals in the community.
- Frequently stands, walks, sit, and climb in performing duties in the office and traveling to off-site meetings.
- Frequently reaches and grasps in using telephones, computers, copy machines, and other office equipment and supplies.
- Frequently lifts and carries up to 5lbs of paperwork, files, and training materials, occasionally up to 40lbs.
- Must have valid state driver’s license by time of hire and be willing to travel significant distances throughout the week. A reliable vehicle for travel is required.
