Registered Nurse (RN) Home Care Case Manager

Key Responsibilities:
- Manage a caseload of home health patients, coordinating and delivering personalized care plans.
- Conduct initial and follow-up assessments to identify the physical, psychosocial, and environmental needs of patients.
- Develop, implement, and evaluate individualized care plans in collaboration with interdisciplinary teams.
- Maintain clear and timely communication with the Clinical Supervisor, physicians, and other team members regarding any changes in patient status or care needs.
- Ensure patient confidentiality and compliance with HIPAA regulations.
- Document all care and communication according to policy and procedure.
Qualifications:
- Current, unrestricted RN license in Massachusetts.
- Minimum of 1 year of experience in a clinical care setting.
- Prior home health or hospice experience is preferred.
- Reliable vehicle, valid driver's license, and current auto insurance.
- Ability to travel 50% within the service area.
- Associate's or Bachelor's Degree in Nursing preferred.
- Familiarity with HomeCare HomeBase is a plus.
