Job Responsibilities:
- Conduct comprehensive assessments of clients' needs and implement individualized care plans
- Provide case management services to individuals and families, including coordinating and monitoring services, skill development
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care
- Assist clients in accessing community resources and support services, provide transportations as needed to remove barriers
- Advocate for clients' rights and ensure their needs are met
- Monitor and evaluate clients' progress towards their goals
- Provide crisis intervention and support during emergencies or challenging situations
- Maintain accurate and up-to-date documentation of client interactions and progress

Requirements:
- Experience in social work or a related field
- Experience in crisis intervention, child welfare, behavioral health, or related fields
- Knowledge of social service programs and community resources
- Ability to assess client needs and develop appropriate care plans
- Communication and interpersonal skills for effective collaboration with clients and professionals
- Experience in supervising or mentoring youth
- Knowledge of Medicare regulations and reimbursement processes is desirable
- Ability to handle children's welfare responsibilities as needed during client interactions
- Strong crisis management skills to effectively handle emergency situations
