We are seeking a dedicated and compassionate Veterans Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals by providing guidance, resources, and advocacy. The Veterans Case Manager will work closely with clients to develop personalized housing plans that address their unique needs and challenges.
Responsibilities
- Conduct comprehensive assessments to determine client needs and eligibility for services.
- Develop, implement, and monitor individualized service plans in collaboration with clients.
- Provide crisis management and intervention support to clients facing immediate challenges.
- Facilitate access to community resources, including behavioral health services, and workforce development programs.
- Collaborate with multidisciplinary teams to ensure holistic support for clients.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
- Advocate for clients within various systems such as Section 8 housing assistance Offer mentoring and support to clients as they navigate their personal challenges.
- Stay informed about best practices in addiction counseling and crisis intervention strategies.
Qualifications
- Bachelor’s degree in social work, psychology, or a related field; relevant experience may be considered in lieu of a degree.
- Strong knowledge of behavioral health principles .
- Experience working with diverse populations and understanding the complexities of social services.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients.
- Proficiency in crisis management techniques and the ability to remain calm under pressure.
- Familiarity with workforce development initiatives is preferred.
- Ability to work independently while also being an effective team player.
- A commitment to ongoing professional development in areas such as addiction counseling and mentoring.
- Knowledge of HMIS --Homeless Management Information System
